STUDENT-ATHLETE MEDICAL INSURANCE
 All student-athletes who participate in a sport within the Department of Athletics at Notre Dame of Maryland University are required to be covered by a Health/Medical Insurance policy through their parent or guardian's policy, their own policy, or with the NDMU Student Health Insurance Plan.  It is the responsibility of the student-athlete and their family to secure and maintain personal health insurance coverage. The Athletic Training Staff must be notified immediately of any changes in this information.

MEDICAL CLEARANCE
All student-athletes must meet the Medical Clearance Requirements prior to participating in Notre Dame of Maryland University Athletics Program. Please understand that no student shall be permitted to try-out, practice, compete, or in any other way participate as a team member until gaining athlete medical clearance from the NDMU Athletic Training Department.

SICKLE CELL TRAIT POLICY
The NCAA has mandated that all student-athletes must be tested for sickle cell trait, show proof of a prior test or sign a waiver releasing the Institution of liability if they decline to be tested. In accordance with this legislation, in order to participate in any Notre Dame of Maryland University sponsored sport, Notre Dame of Maryland University requires that all student-athletes must be tested for sickle cell trait, show proof of a prior test or sign a waiver releasing Notre Dame of Maryland University , its officers, employees, and agents from any and all costs, liability, expense claims, demands or causes of action on account of any loss or personal injury that might result from my non-compliance with the mandate of the NCAA and the requirements of the Notre Dame of Maryland University Athletics Department. These Medical Clearance requirements are specific only to varsity athletic participation and are separate from any other medical forms required by the University in general.
